Trauma-Focused DBT: The Case of Maria

Kirby-Reutter

United States Department of Homeland Security

Key Points

  1. In this clinical case, we focus on how to apply trauma-focused DBT through the case of Maria, a 19-year-old female from Nicaragua.
  2. Maria’s presenting complaint is a chronic history of physical, emotional, and sexual abuse in three stages: she was repeatedly molested as a child, experienced domestic violence by her ex-boyfriend in Nicaragua, and was kidnapped and gang-raped in Mexico.
  3. By the time of this intervention, Maria was detained in an asylum seekers’ facility in the US, and was in her third week of pregnancy due to the gang-rape.

Client Presentation

For this clinical case, exemplifying how treatment using trauma-focused DBT might unfold, the client has been given the name of Maria. She is 19 years old, a Hispanic woman from Nicaragua, and presents with a chronic history of physical, emotional, and sexual abuse.

Precipitants

Maria’s father left the family when she was two years old. Her mother remarried, and her stepfather started molesting Maria weekly when she was five. When intoxicated, he was prone to violent rages, and would beat Maria’s mother in front of her and her three siblings.
